An advanced health care directive is a form that allows you to designate
who you want to make medical decisions on your behalf and gives general
directions about making those decisions.
Your agent does not need to be local, or have a medical mind. I recommend
that you select an agent that gets along well with your friends and family,
listens to your thoughts and concerns carefully, and is willing to stand-up
for what they believe in.
Please note, that this form does not include a
Do Not Resuscitate Order. If you want to prevent emergency personnel from taking life-prolonging
measures, please ask your doctor to fill in this form for you.
I recommend that in addition to naming and agent, clients also give directions
on the following:
- Organ donation
- Pain medication
- End of life decision-making
- Vegetative States
- Severe Dementia
